Hydrochloride 1% mixed with epinephrine to a strength of 1:200,000 is used to inject the skin circumferentially, with a 30-gauge needle. A spinal needle is used to inject the local anesthetic up to the level of the levator muscles at eight points around the anus. Hydrochloride is selected because its action is almost instantaneous in onset, and there is no pain secondary to the chemical contact with tissues. The bupivacaine with ephinephrine is slow-acting and is irritating to the tissues, but its redeeming feature is its long duration of action.
